Getting Started Checklist
The following must take place before services are provided by Alexander& Stance-A Path for Family:
Both parents must contact our office to schedule intake interviews. Face-to-face interviews are conducted separately with each parent.
Orientation to the professional provider process. A face to face orientation, including children over 3 years of age, once it has been determined if we can take your case. This is the time to learn about, discuss and sign the service agreement
Submission of all relevant Court Documentation:
*The original and most recent Court Order and directives
*Signed release of information, covering all parties
*Any police reports regarding the case
*A copy of any Temporary or Permanent Restraining Order
*Any Domestic Violence Orders
* Driver's License
* Copy of Auto Insurance
* A recent picture of the child (children) - (This can be taken by the monitor at time of orientation)
* If there are orders regarding domestic violence a photo will be taken by monitor of the non-custodial parent with that party's vehicle.
